Do you need to give the name of your doctor when you apply for enlistment or at Meps?

I am curious if someone applies for enlistment in the military, would they need to give contact information of their doctor on the enlistment application. It kind of seems like everyone has a doctor, its like going to the dentist but if someone does not routinely see a doctor would they not have to give any contact information on the forms when they apply for enlistment or at MEPS? Also, if someone does not give the contact info of their doctor at meps, would their enlistment stop right there?
